Medical History

Note

I am John Doe, born on March 15, 1985, and I have been experiencing persistent sinus congestion and headaches for the past two weeks. My medical history includes asthma, diagnosed in 2005, and hypertension, diagnosed in 2018. I use an albuterol inhaler as needed for asthma and take 10 mg of lisinopril daily for hypertension. I am allergic to penicillin, which causes a rash, and I had a tonsillectomy in 1992. My father has type 2 diabetes and hypertension, and my mother has asthma and hypothyroidism. I don't smoke, drink alcohol occasionally, work as a software engineer, and exercise regularly. Recently, I've had severe nasal congestion, facial pressure and pain, headaches rated 5-7 out of 10, and a mild fever. I have been advised to increase fluid intake, use a humidifier at night, take an over-the-counter nasal decongestant, and schedule a follow-up appointment in two weeks if my symptoms persist.
